These are the causes of the unsafe acts and conditions. Once the immediate causes of an accident are known, one has to ask why the unsafe act was caused; why this unsafe condition appeared and what failed in the working system that would allow this unsafe act or condition to occur.
The basic causes are the source of symptoms and they correspond mainly to failure in the implementation of the prevention system, and they may be of a personal nature (personal factors) and organization of work (work factors).
For example: The immediate cause of an accident can be the lack of protective clothing and the basic cause may be that the garment is not used because it is uncomfortable. It is therefore essential to try to find and eliminate the basic causes of accidents.
